Care Plan Reviews
Care Plan Reviews today. Care plans are constructed monthly by peers following a person centred approach and with staff support. Midway through the month, they are reviewed. Are things going to the plan, anything else to add and work on? Measuring goals and achievements in the form of a care plan is a big part of what we do here.
